three different goals
The old route looked like one long pet quest. It is really three separate outcomes, and choosing the wrong one wastes a daily attempt.
Temporary goose follower
Complete the treatment successfully. The reward can be a Sturdy Goose Egg or Sickly Goose Egg; use it to summon a temporary follower.
Heal successfully
Goose Slayer + Deranged Egg
Fail or quit the treatment, then defeat the Maniacal or Demon Goose. The kill unlocks Goose Slayer and can drop a Deranged Goose Egg.

Do not farm goose eggs for this. Bonding, mood, photos, interactions, and Companion rewards belong to the separate Version 1.8 system.

Why the screenshot says Shenxian Ferry West
The original Chinese route uses the older area label 神仙渡西. Treat the bridge, riverbank, and sick animal as the reliable landmarks when your English map uses Heaven's Pier or Moonveil Mountain instead.
Track the Exploration activity before searching.
Look for the named Diseased Goose, not a roaming field goose.
Equip Tai Chi before starting only if you plan to force the achievement branch.

Track To Heal or Not to Heal
Open the Qinghe map and look in Moonveil Mountain. The sick goose is on the river path between Blissful Retreat and Heaven's Pier, beside the bridge shown in the source image.

Inspect the Diseased Goose with Wind Sense
Interact with the correct sick goose, then use Wind Sense to open the healing procedure. A normal field goose will not start this activity.

Finish the treatment instead of leaving
Choose the treatment options and clear the healing encounter. If it keeps failing, raise the Healing Career first; current English guidance recommends at least Initiate Tier 4.

Use the Goose Egg
A successful treatment can award a Sturdy Goose Egg or Sickly Goose Egg. Use the hatch or nurse option to summon the goose. This follower is temporary.

Start the same sick-goose treatment
The achievement branch begins at the same bridge-side Diseased Goose. Enter the Wind Sense healing screen normally.

Fail the procedure or confirm the exit
The older route deliberately leaves the treatment screen. Once failure is confirmed, the goose changes into the hostile Maniacal or Demon Goose state.

Lure it toward the nearby deep water
The hostile goose hits hard, and current guidance says it is tougher in multiplayer. Solo is the cleaner attempt if you only want the achievement.

Defeat it; Tai Chi is the shortcut
Use Tai Chi to throw the goose into the water, or defeat it normally. Goose Slayer checks the Demon Goose kill, so the water throw is a practical method rather than a separate reward condition.

Claim Goose Slayer and check the drop
After the kill, claim the Goose Slayer achievement and check for a Deranged Goose Egg. Use the egg if you want its temporary goose follower.
Intentional failure branch
This source image documents the failure branch: inspect the goose, enter treatment, then confirm the exit so the treatment fails. Do not copy this step when you want the successful-healing reward.
After a failed treatment the goose turns hostile. The historical route uses Tai Chi to throw it into the nearby water; the current achievement condition is simply to defeat the Demon Goose.
The older Chinese-client route showed two achievement notifications. The current Global achievement confirmed by English references is Goose Slayer, awarded for defeating the Demon Goose.
The failed-treatment route can leave a Deranged Goose Egg. Use the hatch or nurse option for the temporary follower; consuming the egg is not the route described here.

Use the hatch or nurse option when you want the follower. The older Chinese route reports a bad consumable outcome, but the exact current Global penalty is not confirmed here.

Goose Egg followers are not the persistent Companion system.

The official Version 1.8 explanation describes Companions with mood, bonding, meals, photos, weather reactions, interactions, emotes, and rewards. That is a persistent relationship system. Goose Eggs predate it and create temporary followers.

Raise your Healing Career before retrying. Current guidance recommends Initiate Tier 4 and points to Career Notebooks and Healer Giftboxes as the practical upgrades.
The Demon Goose kills me immediately.
Try the encounter solo, keep the fight beside the water, and equip Tai Chi before forcing the failure. Multiplayer makes the berserk goose tougher.
I failed and the goose is gone.
If the goose was not treated successfully, the activity is reported to reset at 5 AM the next real-world day. Return after reset instead of searching random goose spawns.
My hatched goose disappeared.
That is expected for Goose Egg followers. They are temporary cosmetic companions, not permanent additions to the Version 1.8 Companion roster.
I expected bonding, mood, or photo interactions.
Those features belong to the Version 1.8 Companion system. Use the dedicated Companion guide; hatching a Goose Egg does not convert the follower into that system.
